Support in the community
Through our network of community hospitals, diabetes specialist nurses support adults with type1 and type 2 diabetes. The nurses work with patients to promote a healthy lifestyle and reduce the complications associated with diabetes.
The team who work in the community are available Monday to Friday between 9am and 5pm.
Support for people in hospital
Our diabetes specialist nurses work with Royal Cornwall Hospitals NHS Trust to identify people with diabetes patients in hospital. The nurses provide expert nursing care to patients with diabetes in hospital to support their early discharge and reduce readmissions.
Diabetes specialist nurses educate patients and other healthcare professionals on the management of diabetes in hospital. They also work with GPs and other healthcare professionals.
The nurses are available Monday to Friday at Royal Cornwall Hospital in Truro, West Cornwall Hospital, and St Michael’s Hospital. Cover is also provided over 4-day bank holiday weekends.
