How we will do this

Work with system partners to promote a model of care focused on prevention

We will work in partnership with Public Health to ensure strong clinical representation in system wide prevention initiatives. For example, the Mental Heath Prevention Concordat.

Develop a welcoming and accessible community model of care

Our model of community care will be based around community places that are familiar to people and will prioritise green space, nature and creative approaches. We will co-design care with our neighbourhood partners and those from marginalised sections of our community.

Use our data to focus on outcomes for those from less privileged communities

We must ensure we are able to identify equalities and health inequalities in all of the data that we report and apply a rigorous approach to equality impact assessment when developing and reviewing services.

How will we know we have achieved this?

We will know we have achieved this when:

  • we have a recognised framework of well supported patients and citizens that co-produce services with us and an accreditation scheme to recognise and promote this
  • we continuously and routinely make reasonable adjustments for people with intellectual disability and autism in our services
  • patient feedback reflects the accessibility improvements and is stratified to reflect people from across our communities
  • all of the data we use to monitor our services shows the impact that we are having on equality and health inequalities
