How we will do this

Train our staff in personalised care and shared decision making

We will develop a personalised care and shared decision-making training framework and implementation plan. Consistent supervision and top-ups will ensure the skills are embedded and retained. Patients and people using our services will be supported to use a patient engagement portal, to enable them to guide, report on and manage their own care.

Train and supervise all clinical staff in the correct application of the Mental Capacity Act and Section 117 of the Mental Health Act

All patients whose care is influenced by this legislation will have care delivered in line with their legal rights.

  • Review our Mental Capacity Act training and delivery across the Trust.
  • Train and supervise all clinical staff in the application of the Mental Capacity Act.
  • 100% delivery of Section 117 care planning for those eligible mental health patients.

Deliver excellent end of life care

We will work with system partners to increase the number of people over 70 who have an advanced care plan and ensure that everyone in our care over 50 has an electronic treatment escalation plan. All our community hospital wards will be supported to achieve the Gold Standard Framework for end-of-life care. Talking about the end of life will be as comfortable as talking about any aspect of treatment.

Implement patient-reported outcome and experience measures across all of our services

All services will identify appropriate patient-reported outcome and experience measures and a consistent approach to gathering and inputting this information.

How will we know we have achieved this?

We will know we have achieved this when:

  • all relevant staff have received training and supervision around shared decision making and the Mental Capacity Act
  • our data shows that the Mental Capacity Act is routinely applied, including Deprivation of Liberty Safeguards, and Section 117 care planning is provided to 100% of eligible patients
  • 70% of people over 70 living in Cornwall have an advanced care plan, everyone in our care over 50 has an electronic treatment escalation plan and all of our community hospital wards have achieved the Gold Standard Framework for end-of-life care
  • all of our services present patient-reported outcome and experience measures as part of integrated performance
